Please have the appropriate intake form completed PRIOR to your consultation. Once you have the form completed you can either email it to amanda@mazaherilaw.com or you can fax it to 405-607-4358. You can also print out the form and bring it with you the day of your consultation. If you are unable to complete your intake form prior to your appointment, please arrive 15-30 minutes early to complete. If you have scheduled a Phone Consultation please return your completed form either via fax or email.
